California State Disability Insurance
There is a new way to complete a claim for State Disability Insurance benefits, DE 2501. Effective March 1, 2002, you can complete and print the Claim for State Disability insurance (SDI) benefits, DE 2501, from the Employment Development Department (EDD web site).
You can access the DE 2501 through the SDI web pages as http://www.edd.ca.gov/direp/diadvis2501.htm
